Terms and Conditions

Please read these terms and conditions carefully before applying for the Health Card (hereinafter referred to as “Card”) issued by JRBN Info Media Pvt. Ltd. (hereinafter referred to as “Company”).

  • The Company has not done any medical/health check-up or test with respect to details/information furnished by the applicant. The sole responsibility of the correctness of the particulars furnished lies with the applicant.

  • All the information and data are provided by the applicant and accepted as such on a bonafide basis. No cross-check of information has been done by the Company.

  • Company does not take any responsibility about authenticity/ correctness/ accuracy about the information provided by the applicant.

  • The applicant should contact the Company as soon as possible if the Card is misplaced, lost, stolen, mutilated, not received when due or if he/she suspects that the Card is being used without the applicant’s permission.

  • The Company is not liable or responsible for any loss incurred on the Card prior to the time of reporting of the loss of the Card to the Company and the applicant will be wholly liable for the same. In addition to notifying the Company about the loss or theft of the Card, the Cardholder must report any theft of the Card to the Police and lodge an FIR. The applicant will, however, be liable for all losses when someone obtains and misuses the Card with the applicant’s consent.

  • In case of lost/theft of the Card, the applicant will be duty-bound to report details thereof in writing as soon as possible and not later than 3 (three) days. The applicant shall be solely responsible for the loss incurred by the theft/loss of the Card. The Company shall not assume any responsibility or liability for the damage to the applicant. The Company shall not indemnify the applicant for such damage.

  • If the applicant has acted fraudulently, the applicant will be liable for all losses. If the applicant acts without reasonable care, the applicant may be liable for all losses incurred. This may apply if the applicant fails to follow the safeguards as specified by the Company.

  • Changes in contact details including mobile number, of your card, would be intimated to your new as well as preceding contact details in the system. It is advised to keep your current contact details including, mobile number updated in the Company’s records.

  • The security of the personally identified information is important to the Company. While there is no such thing as “perfect security” on the internet, the Company will take reasonable steps to help ensure the security and safety of the data. However, the applicant understands and agrees that such measures do not guarantee that the use of the Card or the website is invulnerable to all security breaches; and that the Company makes no warranty, guarantee or representation that use of the Card and the website is protected from viruses, security threats or other vulnerabilities.

  • Company does not take any responsibility for the media on which the information has been stored.

  • Company does not take any responsibility about the theft of card/misuse of details mentioned on the card / wrongful access of information by the holder of the Card.

  • The Card is prepared as per the data provided not older than 1 (one) month. The applicant undertakes that the information provided to the Company is not older than 1 (one) month. In case old information is furnished, the Company shall not be held responsible for the loss or damage to the applicant.

  • Company shall not be responsible for any wrong, or incorrect details provided a part of the application form.

  • Whenever the applicant is providing information, the applicant agrees to provide true, accurate, current and complete information; and maintain and promptly update such information to keep it true, accurate, current and complete. If the applicant provides any information that is, or the Company has reasonable ground to suspect that the information is, untrue, inaccurate, not current or incomplete, the Company may without notice suspend or terminate your access to the website and the Card.

  • The applicant further acknowledges that the Company is authorized to share its information with doctors, insurance companies, or any other person if the Company is under any obligation to do so.

  • Company will not be responsible for any medication provided by the medical professionals/any other professionals based on the details provided in the Card.

  • Applicant should update details at regular intervals. The applicant is under obligation to update the information as soon as possible and not later than 15 days of any change in health conditions/ medical history. Any delay in updating the data and consequences arising on account of the same shall be the sole responsibility of the applicant.

  • The Card contains details/information only about the applicant and not of any other person.

  • The Card will be delivered at the address provided by the applicant in the application form.

  • The Company shall not entertain any enquiry in respect of the Card or information/ data stored by the Company after the expiry of the Card Con any ground whatsoever. The Company reserves the right to renew the Card.

  • The Card should not be used other than for the purpose for which it was created. The Card should be renewed only from “JRBN INFO MEDIA PRIVATE LIMITED” and not from any other agency whether or not doing same or similar activities.

  • In case of any dispute or any conflict, AHEMDABAD Court shall have the exclusive iurisdiction.
